Project aim: intensive support provided as per FLOW process above. The project works with more complex clients/circumstances on a longer-term basis, also using a MEAM (make every adult matter) trauma informed TAP approach. Clients have more in-depth assessments due to their history and situation, including CNA (complex needs assessment) and ACE (adverse childhood experiences), they would also be high demand on the partners of High Flow (SWAST, PCN, DPT, RDUH, Police, ASC, NDC, TDC)
